Your time is valuable to us. In an effort to avoid unnecessary delays in your being evaluated by our doctor, we request the following:

• Arrive at least 10 minutes early for your scheduled appointment time. If you are late, you may be given the choice to wait until the doctor is available to see you or we will reschedule you to another day.
• Make sure you have all of your insurance information available, including your insurance card.
• Bring a list of all medications (prescription and nonprescription) you are taking including dosage, i.e. 200mg. Ibuprofen, 3x/day.
• Unless your child is scheduled for the appointment, please do not bring them with you to your appointment.
• Turn off (or set to vibrate) your cell phone during your appointment.
